'Army Form B.2606 (Revised)'; one-piece of white paper machine printed with pink coloured front; information provided is the person's name, sex, height date of birth, date issued is handwritten in blue ballpoint pen and signed by the bearer; stapled in the top right corner on the front is a black and white photographic image of a young man with dark hair, wearing an Army military uniform jacket, on the front left torso is a line of ribbons, collared shirt, a coloured tie; in purple ink stamped on the corner of the photograph is a large circle with a crown in the centre; machine printed on the reverse in black text are the general conditions of issue, dated 1946.
