Concerning record keeping for public health nurses. Back and front covers of the loose-leaf book are black imitation leather. Book is bound with two silver-coloured screws and posts; screw heads are 1 cm in diameter. Numerous examples of forms are pasted into the manual. Section 15, Class Room Health Records with an example of the applicable form, was added at a later time as the index ends with Section 14. Inside the front cover flyleaf is the pencilled notation "Replaced 1950".
